Documents Required for LIC Policy


Following Documents are Required for LIC Policy

1)      Self Attested Photograph
2)      Age Proof: (SSC Certificate, Passport, PAN card, Driving License)
3)      Residence proof (Ration Card, Voter Card, Electricity Bill, Water bill, Telephone bill(BSNL), Passport etc)
4)      Photo ID proof (PAN card, Driving License, Passport, Voter Card, Ration Card )
5)      Income proof (salary slip, form 16, Income tax returns, pension pass book etc) (If total Premium Exceeds one lakh)
